Starting or returning to Secondary / High School can be an overwhelming time for any student but in particular for students who are autistic and attending or returning to secondary / high school.

This Communication Passport is designed for teenagers and their care givers to help support them in the transition. It included detailed topics as a starting point for caregivers and teenagers to fill out information to support their autonomy and inclusion in the educational setting. There is also the topic of "Puberty / Hygiene Information" where it is appropriate to highlight information pertinent for their teachers / support staff to be aware of.

This resource can be printed each year and updated to support your teenager as they age and their support needs change.
